Use this checklist to make an assessment more useful:
- Nominate the management contact, maintenance lead, and site safety contact.
- Define the area, line, assets, or problem in scope.
- Prepare induction, permit, isolation, access, and PPE requirements.
- Collect available asset lists, manuals, drawings, PM schedules, and breakdown records.
- Identify recurring failures, critical spares, temporary repairs, and open actions.
- Confirm whether photographs are permitted and how confidential information will be controlled.
- Make representative technicians, operators, and supervisors available.
- Note any inaccessible, operating, contaminated, or unsafe equipment.
- Agree the expected report, presentation, and action-review process.
Unknown information should be marked for verification rather than guessed.
