  • Who verifies closure
  • Repeat findings that were previously marked complete
Separate the explanations

Questions that improve the diagnosis

  • Is completion different from verification?
  • What evidence proves the defect or risk was addressed?
  • Who has authority to accept closure?
  • Does a repeated finding reopen the original action or create a new one?
Avoid premature action

What not to conclude too early

  • Closing actions on verbal confirmation alone
  • Using percentage complete without evidence of the result
  • Mixing statutory certification with ordinary operational close-out
